Family Practice/Primary Care Physician Assistant

Conviva Senior Primary Care is seeking an experience clinician for our Primary Care Nurse Practitioner/Physician Assistant position in Port Orange, Florida.

As a Clinician with Conviva, you ll have the opportunity take a hands-on approach to addressing patient needs inside the exam room, as well as looking beyond to understand how social determinates of health are impacting health outcomes. A day in the life of a Conviva provider:

• See 12-15 patients per day. Opportunity to spend more time with patients
• Thrive in a value-base care model and improve patient s quality of life
• See a variety in your caseload, from preventative care to chronic conditions
• Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care

Your Accomplishments are Valued:

• Active, unrestricted NP/PA license in the appropriate state
• Current certification as an NP or PA
• Current DEA license or ability to obtain a DEA license
• This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
• Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Preferred Qualifications:

• 2+ Years Experience as a Nurse Practitioner/Physician Assistant
• Familiar with Value Based Care model
• Outpatient Senior Care Experience
• Familiar with HEDIS measures
