Modules are required to be done before start of assignment. Modules take on average 3-4 hours to complete.TIME SPENT COMPLETING THE MODULES IS NOT BILLABLE2 years experience in a healthcare setting requiredRequired certs: BLS & NPI # Also needs a CA OT license HH requires a current driver's license & prof of automobile insurance. We would consider a COTA with home health experience if there are no OT applicants. four visits per day on average; combination of Evals and RevisitsCoverage area includes north-eastern Sac into Folsom

Licenses: OT-CA(Copy needed)
Certifications: OTR-National Board for Certification in Occupational Therapy(Copy needed); BLS-AHA(Copy needed)

Work History: Should not be more than 30 days of gap in entire work history
References: 2 References in the last 12 months
