DESCRIPTION: This is a full-time PERMANENT need in Madison, Wisconsin. Start as soon as credentialed. Provider will work in collaboration with the Hospice and Home Care teams to assess patient need and manage caseload. Will consider recent grads.

• Monday-Friday 8:00AM-4:30PM
  
• No Call, Weekends, Holidays
  
• Recent Graduates Okay
  

REQUIREMENTS:

• Wisconsin License
  
• DEA
  
• BLS
  
• Less than 1 Year Experience
  

COMPENSATION: Competitive rate will be negotiated with client.
